CME Course Description:

Social determinants of health (SDOH) are the conditions in which people are born, live, work, and age that impact health. Unfortunately, many factors outside of the provider’s office play a big role in the health and wellness of patients. This session will focus on a patient story as we identify the social determinants of health that are impacting our patient, discuss the team-based approach to maximize services and care to address the social determinants of health, and conclude with how implementing a value-based care model supports providers in assessing the whole patient to promote positive outcomes.

Prior to assuming this role, Dr. Castro was a staff psychiatrist, Assistant Professor with the Sandra Levine Psychiatry Residency in Atrium Health, Chief of the Department of Psychiatry and the Medical Director of the Behavioral Health Integration Program. Dr. Castro was responsible for the provision of medication management to both inpatient and outpatient psychiatric patients, provided Consult Liaison services to surrounding Atrium Health Facilities. He was responsible for leading the creation and design of an innovative, sustainable, and clinically efficacious Virtual Behavioral Health Integration model. Dr. Castro is responsible for the development of an evidenced based, treatment algorithm for depression and anxiety in the electronic health record and developing an education program for providers and clinical team on the treatment of depression and anxiety within the primary care practice setting. Through the Atrium Health Behavioral Health Integration Program Dr. Castro was able to provide Integration services and support to over three hundred Primary Care Providers and specialists. Dr. Castro directed the clinical supervision for Psychiatric Nurse Practitioners and Clinical Psychologists. His scope of practice included a behavioral health emergency room, adult inpatient for the severe and persistently mentally ill, and adult outpatient for maintenance treatment.
